What the evidence shows
The faster a heart-failure patient reaches a cardiologist after referral, the lower their risk of death and readmission — a large, peer-reviewed cohort shows it directly.

In the same cohort, each additional day of delay raised 1-year mortality, and patients seen after more than 14 days carried a 31% higher adjusted mortality risk. Delay doesn't remove demand — it converts a manageable outpatient problem into an inpatient and ER one.

CHF readmissions dominate inpatient spend. Structured telemonitoring reduces heart-failure hospitalizations vs. usual care (Cochrane, RR 0.71).
Type 2 diabetes
Uncontrolled diabetes drives downstream cost. Telehealth remote monitoring lowers mean HbA1c vs. usual care (BMC Health Serv Res, 2018).
COPD & sleep
COPD exacerbations are preventable and recurring; sleep-disordered breathing is Medicare & Medicaid's most underdiagnosed CHF/COPD comorbidity. Program outcomes available under case study.
