Circadian Health Showcases Pioneering Telehealth Diabetes Study at 2025 American Diabetes Association Scientific Sessions
Virtual-first specialty care provider highlights impact of ongoing virtual support on diabetes self-management and outcomes
West Palm Beach, FL – June 20, 2025 – Circadian Health, the nation’s only virtual-first specialty care provider focused on cardiometabolic conditions, presented new clinical insights at the 2025 American Diabetes Association (ADA) Scientific Sessions this week in Chicago. The accepted poster presentation, “Ongoing Telehealth Support in Diabetes Can Significantly Impact Self-Care,” revealed how structured, remote diabetes management significantly improved patient outcomes, particularly among those impacted by social determinants of health (SDOH).
The study, authored by Sherri L. Horvat, Ana Remigio, Samuel Rios, Ana C. Pena, and Victoria Cardinale, evaluated a pilot program that paired high-risk patients with diabetes-focused advanced practice providers (APPs) and Certified Diabetes Care and Education Specialists (CDCESs) via telehealth. Participants received a cellular-enabled glucose meter, individualized care plans, and consistent virtual follow-ups focused on the ADCES 7 Self-Care Behaviors.
Key Results Presented:
Patients with A1c levels > 9 (n=28) saw a mean reduction of 1.99 percentage points.
The broader group with A1c > 8 (n=67) achieved an average reduction of 0.94 percentage points in just 120 days.
These results underscore the effectiveness of continuous virtual care in improving self-management and glycemic control among patients with complex comorbidities, many of whom face structural barriers to in-person specialty care. The program served populations across multiple regions, including Fresno County, California, where chronic disease burden and access disparities remain significant.
